Number format: this calculator uses German notation throughout — a comma is the decimal separator and a dot groups thousands. “1.234,56” means one thousand two hundred thirty-four euros and fifty-six cents. The figures come from German tax law, so they are shown the way German authorities write them.
Health insurer supplementary rates 2026
On top of the general contribution rate of 14,6 %, every statutory health insurer charges its own supplementary rate (§ 242 SGB V) — employee and employer each bear half. The selection of the largest insurers below is sorted by supplementary rate. One click carries the rate into the gross-to-net calculator.
A snapshot, not a fixed calculation value. As at 10. Juli 2026. Supplementary rates change during the year; only your insurer's own statutes are binding. The official full list of all insurers: GKV-Spitzenverband. The officially determined average (2,9 %, § 242a SGB V) remains the calculators' default.

Total rate = general contribution rate 14,6 % + supplementary rate; as an employee you pay half of it. Source of the individual figures: zusatzbeitrag.net (aggregator of the insurer-specific rates under § 242 SGB V); official full list: GKV-Spitzenverband.
How the figures are calculated
The general contribution rate is the same everywhere
14,6 % under § 241 SGB V, shared between employer and employee. Differences between insurers arise solely through the insurer-specific supplementary rate under § 242 SGB V.
The supplementary rate is the only lever — and it is smaller than it looks
A difference of 1,0 percentage points means roughly 20 € a month for the employee on 4.000 € gross, because the supplementary rate is shared too. That is real money, but it is no reason to ignore what the insurer actually offers.
A dated snapshot, not a calculation parameter
Insurers can change their supplementary rate. So each entry in the list carries its date and its source, and it is expressly not a substantiated calculation value like a figure from a statute.
Example: the range of supplementary rates in the list
| Cheapest insurer (BKK firmus) | 2,18 % |
|---|---|
| Average supplementary rate 2026 | 2,90 % |
| Total contribution at 2,18 % | 16,78 % |
Between the cheapest insurer and the average lie roughly 0,7 percentage points — about 14 € a month for the employee on 4.000 € gross.
What this calculator does not cover
Pretending to be complete is the costliest mistake a financial calculator can make. These points are deliberately absent — either because they need details this page does not ask for, or because they cannot be substantiated in general terms:
- Benefits and extra services. The contribution is only one measure; bonus schemes, subsidies and service quality are not shown here.
- Completeness. The list is curated and covers the largest insurers, not all of them.
- Currency. Changes may fall after the stated date — the insurer itself is the binding source.

Terms used on this page
Each term has its own page with a plain-language explanation, the technical detail and a worked example.
- Children in total — Anyone who has ever had a child pays no childless surcharge to long-term care insurance, for life.
- Children under 25 — For each child from the second to the fifth, the long-term care contribution falls by 0,25 contribution rate points.
- Civil service medical allowance rate (Beihilfebemessungssatz) — The share of medical costs borne by the employer; the remainder is covered by private insurance.
- Insurer-specific supplementary rate (Zusatzbeitrag) — Every health insurer sets it for itself; the published average rate is only a default.
- Type of employment (Beschäftigungsverhältnis) — Determines social security liability as a whole — civil servants, judges and soldiers pay neither pension nor unemployment contributions.
